Exercise for Chronic Disease Management 

At Fortitude Rehabilitation and Conditioning (FRC), our Accredited Exercise Physiologists (AEPs) provide evidence‑based exercise programs to support people living with chronic health conditions. Through Medicare‑subsidised Exercise Physiology services, eligible patients can access tailored programs designed to improve health, mobility, and quality of life.

 

Chronic Disease Management (CDM) Plans

If you have a chronic medical condition (lasting 6+ months), your GP may prepare a Chronic Disease Management Plan (CDM). This can include:

  • GP Management Plan (GPMP): For patients with ongoing conditions such as diabetes, arthritis, asthma, cancer, cardiovascular disease, or stroke.

  • Team Care Arrangements (TCA): For patients with complex needs requiring input from multiple healthcare providers.

With both a GPMP and TCA, you may be eligible for Medicare rebates on up to five allied health sessions per calendar year, including Exercise Physiology.

  • Long COVID and post‑viral fatigue

Our AEPs design safe, personalised programs that:

  • Reduce pain and improve mobility

  • Help you return to daily activities with confidence

For patients with type 2 diabetes, additional group‑based Exercise Physiology sessions may also be available under Medicare.

 

Getting Started with Medicare Rebates

  1. Speak with your GP about a GPMP and TCA referral.

  2. Book your initial assessment at FRC.

  3. Begin your tailored program with ongoing support and progress reviews.
